<p><strong>Remote/Hyrid Working Nurse Manager – Community Complex Care</strong></p><p> <strong>Salary £48,000</strong></p><p><strong> Basingstoke, Reading, Oxford </strong></p><p> <strong>Hybrid: 3 days community-based | 1 day from home | 1 day in office </strong></p><p> <strong>UK Driving Licence & Own Car Required</strong></p><p><br><br>Have you got what it takes to succeed The following information should be read carefully by all candidates.<br></p><p><strong>Sentra Care</strong> are working in partnership with <strong>our client, a recognised leader in the Community Complex Care sector</strong>, to recruit an experienced Clinical Lead to support adults and children with complex healthcare needs in the community.</p><p><br></p><p>This is a highly autonomous, clinically focused role offering a true hybrid working model, with the opportunity to make a meaningful impact on client outcomes while maintaining a healthy work–life balance.</p><p><br></p><p><strong>The Role</strong></p><p>you will be responsible for managing a community-based caseload across <strong>Basingstoke,</strong> <strong>Reading and Oxford</strong>, ensuring the delivery of safe, high-quality, nurse-led care.</p><p>The role will consist of:</p><ul><li><strong>3 days per week in the community</strong>, completing client assessments, staff training, touchpoints, reviews, and clinical oversight</li><li><strong>1 days per week working from home</strong> for care planning, governance, and MDT coordination</li><li><strong>Office Days</strong> for team meetings and training. </li></ul><p><br></p><p>You will work closely with Care Coordinators, operational teams, and senior clinical leadership to ensure robust, client-centred care delivery.</p><p><br></p><p><strong>Key Responsibilities</strong></p><p><strong>Leadership & Collaboration</strong></p><ul><li>Report into the Regional Lead Nurse and work alongside Practice Educators, and Care Coordinators</li><li>Build strong relationships with clients, families, community teams, and specialist clinicians</li><li>Work with central resourcing teams to ensure a skilled and consistent care workforce</li></ul><p><br></p><p><strong>Service Delivery, Quality & Governance</strong></p><ul><li>Create, review, and update person-centred care plans and risk assessments</li><li>Provide visible, hands-on clinical support in the community</li><li>Conduct client reviews at least every 8 weeks</li><li>Participate in the nurse on-call rota</li><li>Escalate clinical and safeguarding concerns appropriately</li><li>Support complaint resolution and clinical investigations</li><li>Ensure compliance with clinical policies and regulatory standards xwzovoh </li></ul><p><br></p><p><strong>Essential Requirements</strong></p><ul><li>NMC Registered Nurse</li><li>Background in ICU/ITU/HDU <strong>or</strong> complex continuing healthcare - <strong>MUST HAVE Recent Tracheostomy & Ventilation exp - within last 6 months </strong></li><li>Right to Work in UK - They cant offer Skilled worker visas at this stage</li><li>Must be based in Either Hampshire, Reading, Oxford or surrounding areas</li><li>Post-qualification experience in acute clinical care</li><li>Experience mentoring, supervising, and developing staff</li><li>Strong communication, analytical, and organisational skills</li><li>Ability to work autonomously and manage a community caseload</li><li><strong>Full UK driving licence and access to your own vehicle</strong></li></ul><br>Remote working/work at home options are available for this role.
